New Patient Registration
We are happy to register patients living within DY1 or DY2 postcodes, or within 1.5 miles of either surgery. We can register patients outside this area but will not provide home visits.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients should have a new patient medical with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. At this appointment, you will be notified of your named accountable GP.
Please contact reception to arrange an appointment for your new patient medical. Note that medical treatment is only available once you have had your new patient medical. Please contact reception for further information.
